This order approves among others the format and content of form 095 – “Request for the registration/exclusion from the Intra-communitarian operator registry”, code MFP 14.13.01.10.11/r.o.i.
The request shall be submitted to the competent fiscal body accompanied by:
an annex regarding the identification details of administrators and associations of the contributor on the date when the registration request is submitted;
the criminal record certificates of administrators and associates, issued for registration in the Registry of intra-communitarian operators. The registration requests by corporations shall be submitted only accompanied by the criminal record certificates of the administrators;
documents issued by prosecution bodies and/or documents issued by the service/judicial department of the general section for public finance in case the criminal record contains penal actions, which states that these actions are not related to intra-communitarian operations.
Caution, contributors have the obligation, within 30 days from the modification of the associate and/or administrator list to notify the fiscal body, by submitting a new annex to the registration request accompanied by the criminal record of the new associates and/or administrators.
